| 内容記述 | Background: To understand the characteristics of stroke patients discharged home from an early stage and to provide an effective rehabilitation program, we retrospectively examined factors affecting the outcome destination of patients admitted to a convalescent ward (either back to their home, or lsewhere, such as a care facility). Specifically, we examined the effect of a home visit by a physical therapist at the time of admission for information gathering. Methods: We included 212 patients aged ≥65 years who were hospitalized for a first cerebral hemorrhage or stroke between December 2019 and ovember 2022. Interview and assessment items at admission were collected from medical records. The significance level was set at <5%, and after univariate analysis comparing the two groups, we conducted multivariate analysis to determine independently associated factors. Results: The discharged-to-home group (87 people) was younger than the group who were discharged elsewhere (125 people), but there was no ignificant sex difference between these groups. However, their rate of home visits by a physical therapist at the time of admission was higher. In addition, items that are indicators of activities of daily living score, muscle strength, swallowing, and nutrition were significantly higher in those discharged to their homes. Binomial logistic regression analysis selected a total score of 3.94 (95% CI: 1.58-9.84) for the admission visit and 1.04 (95% CI: 1.00-1.07) for the functional independence measure exercise items at admission. Conclusion: Home visits by physical therapists at the time of admission and the total score of the functional independence measure motor items were associated with the outcome destination of stroke patients admitted to the convalescent ward in this cohort. This suggests the importance of gathering home-related information and maintaining motor function in the early stages of admission. |
