| 内容記述 | A rehabilitation team approach with emphasis on quality of life was applied to symptoms, such as critical ingestion, swallowing difficulty, and articulation disorder, after post-operative and post-chemoradiotherapy treatment in a patient with head and neck complex multiple cancers. Through the cooperation of the rehabilitation staff regarding nursing, gastrointestinal surgery, dentistry and dietetic services, etc., there was success in the establishment of a nutritional route with improvement of swallowing function and articulation, as well as prevention of pneumonia. It was resulted in house leaving hospital. Although nutrition was attempted via intermittent oro-esophageal tube feeding (OE method), ultimately, intestinal fistula was used. Dent-maxillary prosthesis with palatal augmentation prosthesis (PAP)was created with an observed improvement of swallowing pressure and of opening of the upper esophageal sphincter, and consistencies of oral ingestion was limited to jelly and sips of water. Cooperation between the ingestion/swallowing team and other specialties and professionals was emphasized. | |||||||||

| 内容記述 | 頭頸部多重多発癌による術後の障害と化学放射線療法後の障害のため, 重篤な摂食嚥下障害・構音障害などをきたした症例に対し, 摂食嚥下チームアプローチがなされた. リハビリテーションスタッフのみでなく, 看護師や消化器外科, 歯科, 栄養士などの協力を得られたことから, 栄養摂取ルートの確立, 肺炎の予防, 嚥下機能改善, 構音明瞭度改善を認め, 自宅退院に至った. 栄養摂取はOE法を試みたが, 最終的には腸瘻となった. PAP付き顎義歯を作成し, 嚥下圧の改善, 食道入口部開大に改善を認めた. しかし経口摂取はゼリーと少量の水分にとどまった. 摂食嚥下チームと他の診療科や専門職との連携の重要性が確認された. | |||||||||
